Clinical presentation
- Voluminous isolated comedone on the face or chest that cannot be completely squeezed out
- Mechanical expression of its keratin content always leaves a small hole 2-3mm deep
Histology
- Enlarged, cavernous cavity, shaped like a cave, whose aconthotic epithelial wall sometimes proliferates in the form of multiple small buds in the perilesional connective tissue
Pilar sheath acanthoma is a similar entity, deriving from all the structures of the pilosebaceous follicle and not just the infudibulum
