Omalizumab

Mecanismo de ação
  • Humanized IgG anti-IgE Fc
  • binds free IgE in the blood (exact MOA not known) → quanto maior [IgE] no sangue maior a resposta ao omalizumab
  • Prevents free IgE from binding FcERI on basophils and mast cells
  • Does not bind cell surface IgE/IgE receptors (does not lead to degranulation)
  • Leads to reduced expression of IgE FceRI on basophils and mast cells
Indicações
  • Chronic Idiopathic Urticaria
    • Melhor preditor de resposta: IgE <43
    • Patients >12 years, refractory to H1 antihistamines
    • Dosing is NOT determined by body weight or IgE levels (contrast with asthma)
    • Unclear whether serum IgE levels predict response
  • Asthma ≥ 6 years old
  • Off-label:
    • Bullous pemphigoid (some pats IgE autoAb to BP180), food&latex allergy
    • Other IgE mediated disease (i.e. hyper IgE, Kimura, SLE)
    • Atopic dermatitis (conflicting data)
Contraindicações
  • Hypersensitivity, anaphylaxis (few min - 4 days after administration, monitor 1h after infecting in office)
Dose
  • 300 mg (2 injeções) SQ every 4 weeks
    • Se bem controlado, pode-se aumentar intervalos
    • Se mal controlado pode-se reduzir intervalos (ex. q3 semanas) ou aumentar a dose até 600mg q2 semanas
    • Quando chegar a intervalos de 8 semanas pode se considerar suspender
Dicas de prescrição hospitalar
  • No hospital preferimos seringa, para casa caneta
  • A farmácia só da medicacao para fazer em casa durante 2 meses
Efeitos secundários
  • Injection site reaction (dec. w/ repeat dosing)
  • HA/URI
  • Increased risk of helminth infections
  • The most significant AE is anaphylaxis
    • Blackbox warning
    • Administration in a medical facility, followed by patient observation for a period of time
    • ~ 2 hours after initial injection and 30 minutes for subsequent injections
    • Less likely: serum sickness-like reaction
Monitorização
  • Pedir análises antes da toma e depois ao fim de 3 tomas (ver se tem aumento da IgE, quer dizer que está a fazer efeito)
  • Não precisa de mais
Gravidez
  • Cat B (not approved but may be used if benefits outweigh risks)
  • Pediatrics
    • Licenced ≥12 years
    • Can be used off label <12 yo
