Actinomycosis

  • Etiologia: actinomyces israelii
    • Is a part of normal flora of oral mucosa and also found in GI tract and female genital tract
  • Infections with the bacteria occur secondary to trauma (especially dental procedures)
  • Apresentação clínica
    • Bluish swelling in the jaw that grows to a red tender nodule that forms abscess which drains yellow “sulfur granules” that are clumps of the bacteria
    • May have associated fever, pain, and elevated white count (without lymphadenopathy)
    • Some patients can have lung, GI or pelvic involvement
  • Tratamento
    • Agudo 6 semanas ceftriaxone e clindamicina como se fosse uma osteomyelitis (clinda é bom para anaeróbios)
    • Depois cotrimoxazol ou doxiciclina 100mg 2id
    • Duração de tratamento 6 a 12 meses
