Clinical presentation
- Acute gouty arthritis
- 75% in the first metatarsophalangeal joint - podagra
- Resolves with violaceous hue and desquamation
- Chronic tophaceous gout
- Deposits of monosodium urate
- 10 years after onset of gout
- Firm subcutaneous nodules
- Contour smooth or multilobulated
- May be skin colored, white to yellow or red
- May drain chalky material
- Most common over joints and ear helix (due to underlying cartilage)
- Tophi may improve with normalization of uric acid, nut not always
